Plan of Care
What It Does
A Plan of Care is a program that mixes booked clinic visits with the usual habits and supplements. Instead of only tracking daily habits, staff can add a Service Visit activity — "12 Physical Therapy sessions," for example — and the client's plan then tracks progress toward that visit target the same way it tracks a habit streak. It answers "how far through their course of care is this client?" on one card.
Who uses it: clinicians / care staff (build the plan, book the visit series, close a plan item when care is complete) and clients (see their whole plan — visits + habits — in one place in the app / portal Plan tab).
How It Works
Building the plan (staff)
- Create or edit a program and add activities as usual (habits, supplements).
- Add a Service Visit activity: pick the clinic service and a target number of visits.
- Optionally book the visit series up front so the visits are recognised as belonging to this plan of care. Each booked visit sends a branded confirmation email with its own calendar invite attachment.
Tracking progress (client + staff)
- The Plan tab shows a mixed plan card: the Service Visit item displays an attended / target counter (e.g. "3 of 12 attended"), the next scheduled visit, and flags any unscheduled remaining visits.
- Attending a booked visit automatically advances the counter.
- Staff can close a plan item with a reason (e.g. "goals met," "discharged") when care is done, even if not every target visit was used.
- A flat chronological calendar lays every plan item — visits, habits, supplements — in one time-ordered list so the client sees exactly what's next.
FAQ
- Can a program include actual appointments, not just habits? Yes — add a Service Visit activity with a target number of visits; it tracks attended-vs-target like a habit.
- How do we see how far a client is through their care plan? The Plan tab's plan card shows attended / target, the next visit, and any unscheduled visits.
- What if the client finishes early? Staff can close the plan item with a reason without using every target visit.
